WHO WE ARE:
Community Healthcare Network (CHN) is a not-for-profit organization providing more than 65,000 New Yorkers with primary and behavioral healthcare, dental, nutrition, wellness, and needed support services. Our network is made up of 14 federally qualified health centers throughout Brooklyn, the Bronx, Queens, and Manhattan, along with a fleet of mobile vans that bring health services to underserved people in need throughout New York City. Provides leadership and oversight to the Community Psychiatry Nurse Practitioner Fellowship. Acts as the primary preceptor for the Fellows in the program. Develops and implements fellowship curriculum and training content. and collaborates with consultants and/or experts in the field for program growth and development. Maintains collaborative relationships with external agency partners in the community. Refines and implements methods of program evaluation. Performs assessment and treatment planning for patients in the area of Behavioral Health. Meets regularly with program staff to address clinical issues.
